BSA Medical Forms are needed for all events outside of normal Den and Pack meetings

Follow the link below to the medical forms that are required for campouts, Cub Fun Day and the Nova program. Each person attending (adults, Scouts, siblings) needs to have a medical form on file with the Pack. Please bring them with you to the next event you are attending and give to the event leader.
